To apply online you will need to Right-click and Save As.. this Word file. Complete the information requested and then save it as "SurnameForename.doc". You then attach it to an email to ted@doctorsatevents.com along with a passport-style photo of yourself. Then click the link below to pay via PayPal with a credit or debit card.
 

Clicking the link will open a new window with a form on it. Simply enter the information for your application, then print it out and post it to us with your cheque and a photo.
